Insurance & Patient Care AR

Welcome to East Arkansas Family Health Center, Inc. We look forward to serving you. To prepare for your first visit, please review the insurance and payment information below, as well as what to bring for eligibility.

Insurance/Payment

East Arkansas Family Health Center, Inc. welcomes all private pay, fully and partially insured, Medicaid, and Medicare patients. For patients without insurance, we offer a sliding fee scale based on income and family size to ensure that quality, compassionate health care is readily accessible to everyone.

If you have insurance:
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, it will be necessary to pay at the time of your visit. Payment will also be required for all non-covered services. For your convenience, we accept cash, Visa, MasterCard, and Discover.

If you do not have insurance:
We believe that quality health care should be available to everyone, whether or not you have health insurance. If you are uninsured, you may be eligible for various programs as listed below, if you meet the income requirement:

  • Medicaid
  • ARKids First
  • AR Seniors
  • SOBRA-Pregnant Women
  • Family Planning

Our Medicaid Outstation Worker is available to help you and your family with these programs and other available resources. Help us help you – do not miss out on the health coverage you are eligible for.

Our Fees:
We maintain an established fee schedule for all medical and dental services provided. The fee schedule is reviewed annually by the EAFHC board of directors to ensure sound financial advantage to our patients, and to ensure that the fees are consistent with community standards.

Sliding Fee Scale:
Our sliding fee scale offers significant discounts to individuals and families who qualify based on income and household size.

What to Bring for Eligibility

When you visit any of our East Arkansas Family Health Center locations, you will need to bring the following items with you for payment or to determine your eligibility for a discount on services:

  • Insurance card/proof of insurance
  • If you do not have insurance, you need to bring $50 to your office visit
  • Two forms of identification (i.e., a driver’s license, state ID, social security card, or birth certificate)
  • Proof of income to include one of the following documents:
    • 1040 tax return from the previous year
    • W-2 form (must bring social security cards for all dependents with this form). This form is valid until April 15 of each year.
    • Two of your most recent, consecutive pay stubs
    • Unemployment statement
    • Most recent social security award letter
  • Social Security 1099 form for previous year
  • Food stamp verification form
  • A notarized letter from your employer stating how much you are paid (hourly, weekly, etc.)
  • A notarized letter from a family member stating how much money they give you

New Patient Forms

If you are a new patient, you may download and complete the new patient forms prior to your appointment, which will save you time in the office. Please be sure to bring all forms with you to your first visit, along with any diagnostic films or other test results that may have been ordered or performed by another physician.
